RESOLUTION AUTHORIZING THE CITY OF JONESBORO, ARKANSAS GRANTS AND COMMUNITY DEVELOPMENT DEPARTMENT TO ENTER INTO AN AGREEMENT WITH THE CENTER FOR DISEASE CONTROL, FY2022 CLOSING THE GAP WITH SOCIAL DETERMINANTS OF HEALTH ACCELERATOR PLAN (CDC-RFA-DP22-2210) GRANT

WHEREAS, the City of Jonesboro, Arkansas was awarded the Center for Disease Control (C.D.C.), FY 2022 Closing the Gap with Social Determinants of Health Accelerator Plan (CDC-RFA-DP22-2210) in the amount of $125,000; and
WHEREAS, the City of Jonesboro will provide no local contribution required toward the total project cost of $125,000; and
WHEREAS, the City of Jonesboro will amend the FY2022 budget; and
WHEREAS, the Center for Disease Control (C.D.C.), FY2022 Closing the Gap with Social Determinants of Health Accelerator Plan (CDC-RFA-DP22-2210) funding, will support a planning process to evaluate the City’s parks and recreational facilities in order to develop an Accelerator Plan to better serve residents with disabilities.
NOW THEREFORE BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF JONESBORO, ARKANSAS THAT:
SECTION 1: The City of Jonesboro, Arkansas will enter into agreement with the Center for Disease Control (C.D.C.), to accept the FY2022 Closing the Gap with Social Determinants of Health Accelerator Plan (CDC-RFA-DP22-2210) in order to develop an Accelerator plan to better serve residents with disabilities.
SECTION 2: The Mayor and the City Clerk are hereby authorized by the City Council for the City of Jonesboro, Arkansas to execute all documents necessary to effectuate this application.
