RESOLUTION TO RENEW POLICIES FOR CITY OF JONESBORO EMPLOYEE INSURANCE WITH BLUE CROSS, BLUE SHIELD AND AMERITAS DENTAL FOR 2007

Body

WHEREAS, The City of Jonesboro offers Medical and Dental Insurance for employees; and

WHEREAS, The City of Jonesboro has the authority to renew these policies with the current carriers for a period of up to two years following the bid year;

NOW THEREFORE BE IT RESOLVED BY THE CITY COUNCIL FOR THE CITY OF JONESBORO, ARKANSAS, THAT:

1.                     The Arkansas Blue Cross, Blue Shield contract shall be renewed at a rate of $311.28 per month for single coverage and $668.75 for family coverage. The city will pay 70% of the premium for both single and family coverage.

2.                     The Ameritas Dental contract shall be renewed at a rate of $20.59 for single coverage and $44.68 for family coverage. The city will provide single coverage for all full time employees and if family coverage is desired the employee will pay $44.68 per month.

The Mayor and City Clerk are hereby authorized to execute such documents as are necessary to effectuate these contracts between the City of Jonesboro and Arkansas Blue Cross, Blue Shield and Ameritas.
