RESOLUTION TO RENEW THE CONTRACT WITH USABLE TO PROVIDE LIFE INSURANCE POLICIES TO THE CITY OF JONESBORO EMPLOYEES

WHEREAS, The City of Jonesboro offers a $10,000.00 Life Insurance Policy to all full time employees of the City; and

WHEREAS, USAble has proposed to provide the same coverage as last year with an increase in premium;

NOW THEREFORE BE IT RESOLVED BY THE CITY COUNCIL, FOR THE CITY OF JONESBORO, ARKANSAS, that:

1.                     The coverage provided by USAble be extended as recommended by the Finance Committee, at $.03 per 1000 increase in premium.

2.                     The effective dates of coverage shall be January 1, 2009, through December 31, 2009.
