RESOLUTION TO RENEW THE CONTRACT WITH UNUM PROVIDENT TO PROVIDE SHORT TERM AND LONG TERM DISABILITY TO CITY OF JONESBORO EMPLOYEES

WHEREAS, The City of Jonesboro offers Short Term and Long Term Disability as a voluntary benefit for employees; and
WHEREAS, Unum Provident has proposed to provide the same coverage as last year with no increase in premium;
NOW THEREFORE BE IT RESOLVED BY THE CITY COUNCIL, FOR THE CITY OF JONESBORO, ARKANSAS, that:
1. The coverage provided by Unum Provident be extended for the second year of the three (3) year renewable contract, as recommended by the Finance Committee, at no increase in premium.
2. The effective dates of coverage shall be November 1, 2006 through October 31, 2007.
