RESOLUTION TO ACCEPT BIDS FROM ARKANSAS BLUE CROSS & BLUE SHIELD AND AMERITAS FOR MEDICAL AND DENTAL INSURANCE FOR CITY OF JONESBORO EMPLOYEES

WHEREAS, the City of Jonesboro offers Medical and Dental Insurance for employees; and

WHEREAS, the City of Jonesboro advertised for quotes on Medical and Dental coverage for its employees;

NOW THEREFORE BE IT RESOLVED BY THE CITY COUNCIL FOR THE CITY OF JONESBORO, ARKANSAS THAT:

1.  The bid from Arkansas Blue Cross & Blue Shield in the amount of $285.58 per month, per employee for single medical coverage and $613.53 per month, per employee for dependant coverage shall be accepted for the plan year 2006.

2.  The bid from Ameritas, Value Source Plan, in the amount of $18.94 for single dental coverage and $60.12 for dependant dental coverage shall be accepted for the plan year 2006.

The Mayor and City Clerk are hereby authorized to execute such documents as are necessary to effectuate this contract between the City of Jonesboro and Arkansas Blue Cross & Blue Shield, and Ameritas.

PASSED AND ADOPTED THIS 6^th Day of December, 2005.
