RESOLUTION ACCEPTING ARKANSAS MUNICIPAL LEAGUE INSURANCE PLAN

WHEREAS, the Arkansas Municipal League has established a group insurance plan underwritten by the Washington National Insurance Company providing for insurance coverage for officials, employees and their dependents;
WHEREAS, it is deemed to be the best interest of the citizens living in our city that we secure such insurance coverage for the officials and employees thereof,
NOW, THEREFORE, BE IT RESOSLVED by the City Council of the City of Jonesboro, Arkansas, that the Mayor and City Clerk be, and the same are, hereby authorized to secure such a group insurance plan from the Arkansas Municipal League to give death benefits surgical, hospitalization, and other sickness benefits for its officials and employees and to the dependents of such officials and employees as they may elect to provide such benefits for their dependents.
The Mayor and City Clerk of this municipality are, and the same are, hereby authorized to pay to the Arkansas Municipal League under the terms of the master policy, which during the initial year will be as follows:
(1) Full life insurance premium for employee not to exceed $1.15 per month. (Employee full premium $4.00 plus dependents of $5.80.)
That the council does hereby authorize the establishment of a system or payroll deductions from the salary of employee who elect to include their dependents in the program whereby their dependents portion of the premium payments required by the program may be paid.
That the officials of the municipality do all things necessary to carry out fully the terms and conditions of said insurance policies and the plan of group insurance thus established; and that a signed copy of this resolution shall be forwarded to the Arkansas Municipal League, 416 Maple Street, North Little Rock, Arkansas.
That this municipality, become a participant in the above described plan effective the 1st day of April, 1957.
