A RESOLUTION TO APPROVE PROVIDER SERVICE AGREEMENT BETWEEN JONESBORO ECONOMICAL TRANSPORTATION SYSTEM AND MICHAEL H. TEAGUE, MSW, LADAC, CCS

WHEREAS, the City of Jonesboro has agreed to renew JETS’ service agreement for professional services with Michael H. Teague, MSW, LADAS, CCS for a value of $1,000.00 annually.

 

WHEREAS, Michael H. Teague, MSW ,LADAS, CCS has acquired, and maintains, acceptable credentials to perform the duties of a Substance Abuse Professional (SAP) as set forth by the United States Department of Transportation (DOT) guidelines.

 

WHEREAS, the City of Jonesboro JETS Department is regulated by the United States Department of Transportation and requires the services of a SAP for referral purposes.

 

WHEREAS, the Transportation Coordinator and the Community Transportation Advisory Board has determined that the selection was made based on recommendation from the Arkansas Transportation Administration to use Michael H. Teague.

 

WHEREAS, Michael H. Teague is recognized by the DOT as the only authorized Substance Abuse Professional in the Jonesboro, Arkansas city limits.

 

WHEREAS, a copy of the Provider Service Agreement is attached.

 

WHEREAS, the City Attorney has reviewed and approved the Provider Service Agreement.

 

NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF JONESBORO THAT the Mayor and City Clerk are authorized to enter into this agreement for services with Michael H. Teague, MSW, LADAS, CCS as a Substance Abuse Professional.
